Veterinary Telehealth Informed Consent

Who is asking for your consent

This consent is between you, the owner or authorized caretaker of the pet named in your Everfur account ("you"), and the licensed veterinarian who will see your pet through Everfur (your "Veterinary Provider"). Everfur (Strand Health Inc.) provides the app, scheduling, video, messaging, records, and payment tools your Veterinary Provider uses and is referred to below as the "Platform." Everfur is not a veterinary practice and does not practice veterinary medicine.

Your Veterinary Provider's name, the states in which they are licensed, their license numbers, and how to verify a license are shown in the app when you book and on everfur.com/service-availability. Your Veterinary Provider's contact information for follow up is the Everfur app messaging feature and hello@everfur.com.

What telehealth is, and the three kinds of service you may receive

Veterinary telehealth uses video, audio, images, messaging, and stored records so that a veterinarian can evaluate and care for your pet without an in person visit. Depending on the law of the state in which your pet is physically located at the time of the Visit, you may receive one of the following, and the app will tell you which applies before you book:

  • Telemedicine. A licensed Veterinary Provider examines your pet by live video, establishes a veterinarian client patient relationship ("VCPR") where state law allows this to be done by telehealth, and may diagnose, recommend treatment, and prescribe medication (subject to the Prescription & Pharmacy Policy).
  • Teletriage and consultation without prescriptions. A licensed Veterinary Provider assesses your pet's situation, advises whether and how urgently your pet should be seen in person, and gives general recommendations, but does not establish a VCPR and does not diagnose, treat, or prescribe. This applies in states that do not permit a VCPR to be established by telehealth.

How a VCPR is established

Where state law permits, your Veterinary Provider establishes a VCPR by examining your pet through a live, synchronous audio video Visit in which the Veterinary Provider can see and hear your pet, reviewing your pet's history and any available records, and taking responsibility for medical judgments about your pet. A VCPR cannot be established by questionnaire, by chat alone, or by an audio only call. Some states do not permit a VCPR to be established by telehealth at all; in those states your Veterinary Provider can provide teletriage only, and any prescription must come from a veterinarian who has examined your pet in person. Once a VCPR exists, follow up care may be provided by messaging, phone, or video when your Veterinary Provider judges that clinically appropriate. A VCPR established by telehealth expires under the rules of your state and a further examination may be required before care or prescriptions continue.

Your pet's location

Telehealth is regulated by the state in which the animal is located during the Visit. You must tell us truthfully where your pet is at the time of each Visit, and your Veterinary Provider will confirm it at the start. If your pet is in a state where your Veterinary Provider is not licensed or where the requested service is not permitted, the Visit will be limited to what that state allows or rescheduled with a different Veterinary Provider.

Benefits and limitations

Telehealth can give you faster access to a veterinarian, avoid stressful travel for your pet, and allow your Veterinary Provider to see your pet in its home environment. It also has real limits. Your Veterinary Provider cannot touch, palpate, auscultate, or weigh your pet, cannot take samples, and cannot perform diagnostics such as bloodwork, imaging, or cytology. Video, lighting, and connection quality limit what can be seen. Information you do not provide, or provide inaccurately, cannot be taken into account. Because of these limits your Veterinary Provider may not be able to diagnose or treat a condition by telehealth and may refer you to in person care. Telehealth is subject to the same standard of care as in person veterinary care, and you always have the option of choosing an in person visit with a veterinarian instead.

Emergencies

Telehealth is not for emergencies. If your pet is having trouble breathing, has collapsed, is having a seizure, has been hit by a car, has ingested something toxic, has severe bleeding, a distended abdomen, or cannot urinate, or you believe it needs immediate care, do not book a Visit: go to the nearest emergency animal hospital or call it now. Your Veterinary Provider will tell you if a Visit reveals something that needs urgent in person care and will help you find a nearby veterinarian or emergency facility. Everfur can show you nearby emergency hospitals in the app. Animal poison control lines: ASPCA Animal Poison Control Center (888) 426-4435 and Pet Poison Helpline (855) 764-7661; both may charge a consultation fee.

Prescriptions

If your Veterinary Provider establishes a VCPR and decides that medication is appropriate, they may issue a prescription within the limits of your state's law. You are free to have any prescription filled at the pharmacy of your choice; on request your Veterinary Provider will give you a written or electronic prescription at no charge, and some prescription drugs or medications may be available at a pharmacy near you. Everfur Veterinary Providers do not prescribe controlled substances. Some medications may be prescribed for a use, dose, or species not listed on the label ("extra label" use); your Veterinary Provider will discuss the risks and benefits with you before doing so. A prescription is a clinical decision, and Membership does not entitle you to any medication. See the Prescription & Pharmacy Policy at everfur.com/pharmacy-policy.

Financial relationships. Your Veterinary Provider is compensated by Everfur for Visits. Everfur has a commercial relationship with its pharmacy partner, Mixlab, Inc., and earns revenue when prescriptions are filled through Everfur. [In states where the 2% prescription revenue share to Veterinary Providers applies, add: In your state your Veterinary Provider may also receive compensation from Everfur that varies with prescriptions filled through Everfur.] These arrangements never affect whether a prescription is issued, and you may obtain your prescription from any pharmacy.

Recording, transcripts, and AI tools

With your consent, Visits are recorded (audio and video) and transcribed so that your pet's medical record is accurate, your Veterinary Provider can review the Visit, and Everfur can prepare a post Visit summary for your Veterinary Provider's approval. Recordings are stored securely, are seen only by your Veterinary Provider, Everfur's Medical Director, and a small number of authorized personnel for quality and safety review, and are kept for the period described in the Everfur Privacy Policy. You may decline recording and still receive care; in states that require every participant's consent to a recording, no recording is made unless everyone present agrees. Recordings are not used to train artificial intelligence models unless you give a separate, optional consent in your account settings. You may not record a Visit yourself without your Veterinary Provider's agreement.

Everfur uses artificial intelligence to help you complete intake before a Visit, to assemble your pet's history for your Veterinary Provider, to draft summaries and record entries for your Veterinary Provider's review, and to run safety checks on prescriptions. The AI assistant is clearly labeled and is not a veterinarian. Nothing generated by the AI is a diagnosis or treatment of your pet until your Veterinary Provider has reviewed and adopted it. Automated tools can make mistakes, and your Veterinary Provider remains responsible for all clinical decisions.

Privacy, security, and communications

Your pet's medical information is protected by the veterinary confidentiality laws of your state and by the Everfur Privacy Policy (everfur.com/privacy). Video Visits are encrypted in transit. Electronic communications carry risks: a connection can fail or be interrupted, a message can be misdirected, and email and text messages are not encrypted end to end. If the connection fails during a Visit, your Veterinary Provider will contact you through the app, by the phone number in your account, or by email to continue or reschedule. By providing your phone number and email you agree that your Veterinary Provider and Everfur may use them to communicate with you about your pet's care, including reminders, results, and follow up questions; you can change these preferences in your account settings.

Your medical records

Your Veterinary Provider creates a medical record for every Visit. You may view and download it in the app at any time and request a copy or summary at hello@everfur.com; copies are provided within five (5) business days, or sooner if your pet needs urgent care elsewhere, at no charge. You may ask that the record be sent to another veterinarian. Medical records are retained for the period required by the law of the state in which care was provided and are not deleted when you close your Everfur account.

Fees, scheduling, and cancellations

Visit fees, what your Membership includes, additional Visit pricing, scheduling, late cancellation, and no show rules are set out in the Everfur Terms of Service, Section 6.6, and refunds in Section 6.7. Your Veterinary Provider does not charge you directly.

Your rights

You may withdraw this consent and stop using telehealth at any time by notifying us at hello@everfur.com or in the app; withdrawal does not affect care already provided. You may choose in person care at any time. You may exclude anyone other than your Veterinary Provider from the Visit. You may ask questions about anything in this consent before you agree. You may request a different Veterinary Provider. If you have a concern about the care your pet received, contact hello@everfur.com or follow the complaint process at everfur.com/complaints, which also explains how to contact the veterinary licensing board in your state.

Acknowledgment

By checking the box when you book a Visit, you confirm that: you are at least 18 years old and are the owner of the pet or a person authorized to consent to its care; you have read and understand this consent, including the limits of telehealth, the emergency instructions, the prescription and pharmacy choice information, the financial disclosures, and the recording and AI disclosures; the information you provide about your pet, including its location, is accurate and complete; you understand that telehealth may not be appropriate for every condition and that your Veterinary Provider may refer your pet to in person care; and you consent to receive veterinary telehealth services from your Veterinary Provider through the Everfur platform under the terms above and under any state addendum that applies to your pet's location.

Booking checkbox text: "I have read and agree to the Veterinary Telehealth Informed Consent, and I confirm my pet will be located in the state I selected during this Visit."

Recording checkbox text (separate, pre checked only where a single party consent state is selected, otherwise unchecked): "I consent to this Visit being recorded and transcribed for my pet's medical record and quality review."

State Addendum: California

This addendum applies when your pet is located in California during a Visit and supplements the consent above. It reflects California Business and Professions Code Section 4826.6.

Only a veterinarian holding a current California license may provide veterinary medicine by telehealth to an animal located in California, and your Veterinary Provider for a California Visit is California licensed. Your Veterinary Provider's name, contact information, and California license number are shown to you before the Visit and are available at everfur.com/service-availability.

A VCPR for a California pet is established by telehealth only through a synchronous audio video examination; it cannot be established by audio only communication or by a questionnaire. Your Veterinary Provider will obtain and review your pet's relevant medical history and any available records before providing care.

The same standard of care applies to veterinary services provided by telehealth as to in person services. You have the option to choose an in person visit from a veterinarian at any time. If your pet has an adverse reaction to treatment, or the telehealth connection fails, contact your Veterinary Provider through the Everfur app or at hello@everfur.com; in an emergency go to the nearest emergency animal hospital. Your Veterinary Provider is familiar with, and can provide you with a list of, nearby veterinarians who may be able to see your pet in person, including emergency facilities.

Prescriptions after a telehealth examination: a prescription may not exceed six (6) months from the date your Veterinary Provider examined your pet or prescribed the drug, and a further examination is required before it can be refilled beyond that period. Antimicrobial drugs may be prescribed by telehealth for no more than fourteen (14) days of treatment; further antimicrobial prescriptions require an in person examination. Controlled substances and xylazine cannot be prescribed by telehealth. Some prescription drugs or medications may be available at a pharmacy, and your Veterinary Provider will provide a written prescription that you may fill at the pharmacy of your choice. No medication will be prescribed by telehealth for a horse engaged in racing or training at a facility under the jurisdiction of the California Horse Racing Board.

Your Veterinary Provider keeps a medical record of every telehealth Visit and will make a copy or summary available to you on request. Confidential information about you and your pet is protected under Business and Professions Code Section 4857. Complaints about a veterinarian may be made to the California Veterinary Medical Board, 1747 North Market Boulevard, Suite 230, Sacramento, CA 95834, vmb.ca.gov.
